A simple phone call to us at 540-268-8804 or email live@spiritsrest.org starts the process. We accept referrals directly from families, hospice staff, doctors, and hospitals. However, the final referral must come from your hospice or palliative care agency to which you and your loved one are currently assigned. If you are not already affiliated with a hospice or palliative care agency, talk with your family physician to start the process.

After the initial phone call, a meeting will be set up with the resident and their family for a tour and to gather any further information needed to assure that we can meet your needs before a decision is reached.

Initial eligibility requirements to become a resident at Spirit’s Rest:

• Adult 18 years of age or older
• Signed on with a local hospice or palliative care agency
• Have a signed DNR (Do Not Resuscitate) order
• Residents must not have any behavioral needs that would disrupt others in the home
• Ability to cover costs (link to FAQ costs) or qualify for financial assistance
• Once mutually agreed that Spirit’s Rest is the right choice for you, a private room will be offered as soon as available. Your hospice/palliative care agency will arrange for your transportation to our home.
